  1. 1. TWO WAYS TO ENTER: * INTERNET: Visit www.cinemax.com from 12:01 PM (ET) on 4/7/03 through 11:59 AM (ET) on 5/12/03 and follow the on-screen directions to enter the required information. Limit one online entry per person/e-mail address. * MAIL: On an Official Entry Form, available while supplies last on special displays at participating retailers, or on a plain piece of 3" x 5" paper, hand print your complete name, street address (no P.O. Boxes), city, state, zip code, date of birth, plus (optional) daytime and evening telephone numbers. (If you do not have a telephone, indicate "No Phone." Failure to comply with this or any other official rule, will result in disqualification.) Mail your entry in a hand-addressed, business-size (#10) envelope with first-class postage affixed, to: Cinemax "The Ultimate in Movie Entertainment" Sweepstakes, P.O. Box 5445, Blair, NE 68009-5445, to be received by 5/19/03. Limit one entry per mailing envelope. No mechanically reproduced entries permitted. Illegible entries are void. Not responsible for lost, late, mutilated, postage due, misdirected entries or entries not received in time for the random drawing.

  2. 2. RANDOM DRAWING: Winner will be selected in a random drawing to be conducted on or about 5/20/03, from among all eligible internet and mailed entries received by D.L. Blair, Inc., an independent judging organization whose decisions are final. Winner will be notified by phone/mail. Odds of winning will depend upon the total number of eligible internet and mailed entries received.

  3. 3. PRIZE & APPROXIMATE RETAIL VALUE: (1) Grand Prize - Winner's choice of either: a.) A 5-day/4-night trip for two to Seattle, Washington from June 12, 2003 through June 16, 2003 to attend the last four days of the Seattle International Film Festival which includes the closing night screening of a Cinemax sponsored film. Prize includes round-trip coach air transportation to/from the major commercial airport closest to winner's home, hotel accommodations at the W Hotel (double occupancy), one dinner for two at Palomino Restaurant (price of dinner not to exceed $100), one dinner for two at Cutters Boathouse Restaurant (price of dinner not to exceed $100), VIP access badges to the Seattle International Film Festival for winner and guest, and $400 spending money for winner (ARV $2,900); OR b.) a check in the amount of $1,000. All travelers must agree to depart and return on dates specified by the Sponsor. Prize consists of only those items specifically listed as part of the prize.

  4. 4. GENERAL RULES: Open only to U.S. residents who, as of 5/19/03, are 21 years of age or older. Employees of Home Box Office, Cinemax, The Seattle International Film Festival, Seattlefilm.com, their affiliates, subsidiaries, advertising and promotion agencies and the immediate family members and/or those living in the same household of each are not eligible. Void where prohibited by law. No substitution or transfer of prize by winner permitted. All federal, state and local taxes are the sole responsibility of winner. All federal, state and local laws and regulations apply. Sponsor is not responsible for incomplete, lost, late, damaged, illegible or misdirected e-mail or for any technical problems, malfunctions of any telephone lines, computer systems, servers, providers, hardware/software, lost or unavailable network connections or failed, incomplete, garbled or delayed computer transmission or any combination thereof. The Sponsor is not responsible for any damage to any participant's computer system/software related to or resulting from participation or downloading any materials in this promotion. CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAW. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW. If for any reason the promotion is canceled, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures or any other causes outside control of Sponsor which impair or affect the administration, security, fairness, integrity, or proper conduct of this promotion, the Sponsor reserves the right at its sole discretion to disqualify any individual who tampers with the entry process and to cancel, modify, terminate or suspend the internet portion of the sweepstakes. In the event of termination of the internet portion, a notice will be posted and a random drawing will be held from all eligible entries received prior to the time of termination and all eligible mailed entries received by 5/19/03. Potential winner must sign and return an Affidavit of Eligibility/Release of Liability within 5 days of attempted notification. Noncompliance within this time period may result in disqualification and an alternate may be selected. Return of any prize/prize notification as undeliverable will result in disqualification and an alternate will be selected. All travelers must execute a Release of Liability prior to ticketing and must possess required travel documents (i.e., valid photo ID) prior to departure. If a minor is selected as the guest of the Grand Prize winner, winner must be his/her parent or legal guardian and must sign the Release of Liability on minor's behalf. Home Box Office reserves the right to substitute prize of equal or greater value. Acceptance of prize constitutes permission to the Sponsor and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. By accepting prize, winner agrees to hold Sponsor harmless against any and all claims and liability arising out of use of prize. Winner assumes all liability for any injury or damage caused, or claimed to be caused by participation in this promotion or use or redemption of any prize. Sponsor is not responsible for typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of the prize.
